Kerala Govt. is issuing an IDENTITY CARD for all "PRAVASIS".
Please use the attached APPLICATION FORMS for the subject matter. Following are the checklist /documents to be attached.
v Application signed by yourself / or spouse/ or close relative.
v Certified by the Panchayath / Ward Member.
v Self attested Passport copy, Visa Copy.
v Ration Card copy (if).
v 2 Stamp size photos.
v Application fees 200.
The completed application form has to be submitted in the Panchayath Office for registration. Application form is attached.
